NOTICE PLEASE READ:
Fair credit reporting act (fcra) regulations require that to open an account with us, we must be provided with a copy your company letterhead, business license #, a brief description of the type of business in which you are engaged and a copy of your business or corporation certificate. This is a one-time procedure and the information can be faxed to us at 212-248-5619.

Exhibit A is also a requirement and essentially tells us that you agree to abide by the FCRA. Once the procedures have been complied with, you will be assigned an account number to be used when ordering reports.
The following is a brief outline of some of the forms you will need and which can be obtained from this site:
DISCLOSURE FORM and BACKGROUND SCREENING AUTHORIZATION FORM:
Before you can order a report from us, you must have this document signed by the applicant. Retain a copy for your file and fax a copy to us along with the order form and the applicant’s resume or job application.
This form must be on a separate page and be #12 font as determined by congress under the fcra.
ORDER FORM:
This is a standard document used to request a report. It tells us what type of investigation you need and where to send the report when completed.
Check the boxes on the left for those items you want investigated. Any special requests should be so indicated.
Fax the order form to us at 212-248-5619 along with a copy of the application of the person to be checked and be sure to include the signed disclosure/authorization from the applicant.
Make sure that the order form contains your name, company name, address, telephone # and fax #. If it is not a secure fax, please make a note of that.

An adverse action notification only comes into play should you decide not to hire someone based in whole or in part on our report.
Essentially it means that if you decide, based on information in our report, that you will not hire or plan to terminate the subject, you must give the subject written notice of your plans along with a copy of the report and give the subject a ‘reasonable time’ to dispute the information in the report. The law did not define reasonable, but a few days is probably sufficient. You may want to check with counsel on that.
